You may also need to add this to your input chain for centos-4 (as the new 2.6 kernel uses an iptables that uses STATES):

$IPTABLES -A INPUT -m state --state ESTABLISHED,RELATED -j ACCEPT

If the problem still persists, I would recomend you log everything before you finally drop them at the end:

$IPTABLES -A OUTPUT -j LOG --log-prefix "OUTGOING-FAILED: "
$IPTABLES -A INPUT -j LOG --log-prefix "INCOMING-FAILED: "

And check your messages log for the -FAILED prefixes when you boot up.
